Referral
consists of a short informal interview
Following the referral you will be placed on the waiting list
for a full assessment. You will be sent an appointment for
your assessment. |

| What is an
assessment ? |
| |
| An assessment is
an interview to explore issues around your substance misuse/
dependence. All decisions about treatment are based on each
individuals assessment and are negotiated between yourself,
your keyworker and the doctor, to work towards your stated
goals. We always ask for a urine sample at assessments and
reviews. You will be seen for assessment within 4 weeks following
referral. |

next ? |

| You will be placed
on a waiting list for a space in service. As soon as it is
your turn, you will be sent a doctor's appointment. |

| What happens
when I am in service ? |
| |
| You will be given a
keyworker who will help create, monitor and review your treatment
plan, and deal with any treatment issues that come up. |

confidentiality ? |

Community Drug Service recognises that confidentiality is
of great importance to clients. You can therefore expect that:

- We do not share information that you give
us with anyone outside the agency without your permission.*
Occasionally the service is required to give information
to outside agencies, i.e. Probation, who may require information
for court reports. However, we will always attempt to
discuss this with you first.
- Information kept about you will only be used as necessary
to provide you with a good service.
- Your records and case notes will be stored safely on
site and you have theright to apply to see them.
- Your GP will always be informed of your prescription.
